fix(server): Tailscale Serve keeps retrying after a failed start instead of staying off - #23605
On Linux without Tailscale operator permission, t3 serve --tailscale-serve logs a warning and reports ready, but remains loopback-only. Correcting the refusal does not publish the server until it is restarted. Desktop's Tailscale HTTPS setting uses the same setup path.
Change
Run Serve setup in a scoped background task after activation. Retry with exponential backoff capped at 30 seconds until setup succeeds. The warning gives the existing diagnostic's remedy and explains that retries continue; identical failures subsequently log at debug level.
Shutdown stops retries and waits for an in-flight setup to report success or reach its existing command timeout, then disables Serve only after acknowledged success. Setup and success recording use the same uninterruptible boundary as main. Subprocess cleanup can extend shutdown beyond the command timeout. An attempt that never succeeds does not authorize disabling an existing mapping. Startup remains available so desktop users can recover through Settings.
cd apps/server && ../../node_modules/.bin/vp test run src/tailscaleServe.test.ts: 6/6 pass. Covers recovery without restart, capped backoff, cleanup after success, shutdown during backoff, an unanswered attempt reaching its command timeout without disabling Serve, and shutdown while successful acknowledgement is pending.
The ownership assertion first failed at f0607aae4c: expected zero disable calls, received one. Removing the interrupt ownership assignment in 47213bfe36 makes it pass.
The delayed-acknowledgement test first failed at 3efa332024: expected one disable call, received zero. Restoring the acquisition boundary makes it pass.
Targeted lint and formatting checks pass for the changed files. git diff --check passes.
Server-package vp run typecheck: exit 0; no TypeScript errors (existing Effect suggestions reported).
Two fresh independent full-diff reviews, GPT-6.1 and Claude Opus 5.5: no confirmed findings.
Server-only change; visual fixture matrix is empty.
cannot-verify server-runtime: fresh actual-server proof of delayed acknowledgement cleanup (A1) and timeout preservation of an existing mapping (A2) could not be completed on this VM. Two A1 startup attempts were interrupted or stalled before readiness and before any fake-CLI calls; A2 was not started. These attempts prove neither row. All captured process groups were stopped and verified absent; no actual tailnet state was changed. The six focused tests cover both behaviors through the service seam. Earlier rapid-shutdown evidence at bc8cb10936 is stale and is not proof of this head. Independent pass-2 proof audit: INSUFFICIENT for these runtime rows.
Historical fixture reproduction: driver. Unverified: real-daemon recovery on the current head (historical evidence above), actual Electron toggle, macOS login timing, Windows, and real logged-out stderr (the login fixture uses a fake CLI).
Models and harness: Claude Opus 5.5 and GPT-6.1 via Claude Code and Codex in T3 Code; original review also used Claude Sonnet 5.5.
A refused first `tailscale serve` leaves Serve off until restart, and
shutdown then never disables it. These tests fail until the layer
retries.

A refused `tailscale serve` at startup (no operator permission, HTTPS
certs off, Tailscale still starting at login) logged one warning and left
Serve off until restart. The server still starts, but now retries in the
background with capped exponential backoff (1s up to 30s), warns with a
remedy keyed off the CLI diagnostic, and only disables Serve on shutdown
if an attempt succeeded.
Fixes#16238

…tdown
Run every attempt, the first included, in one forked retry, so the first
retry waits for the backoff instead of firing immediately. An interrupted
attempt is no longer uninterruptible; shutdown instead assumes it may have
configured Serve and turns it off. The operator remedy is Linux-only.

Macroscope's review found this PR requires human approval — This PR changes Tailscale Serve from a single startup attempt to a background retry and shutdown-cleanup lifecycle that can affect remote reachability and server termination. Unresolved comments identify cases where another mapping could be disabled, an applied mapping could be left behind, or shutdown could wait on an unbounded child process.

Walkthrough
The server now delegates Tailscale Serve setup and cleanup to a dedicated layer. The layer retries failed setup attempts, logs remediation guidance, and disables Serve after successful setup during shutdown. Tests cover retry timing and shutdown behavior.
The layer waits for activation and retries Serve configuration with a capped schedule. It provides remediation guidance, adjusts log levels for repeated failures, and disables Serve during shutdown after successful setup. Tests cover retry timing, later success, repeated failures, and a hanging attempt.
Server integration apps/server/src/server.ts
The server passes the configured port and activation signals to TailscaleServe.layer instead of configuring and disabling Serve inline.

Explanation
The title "fix(server): Tailscale Serve keeps retrying after a failed start instead of staying off" matches the main change. The changeset adds a layer in tailscaleServe.ts that retries Serve configuration with capped exponential backoff after activation, and server.ts is rewired to use it. The title is specific and uses a conventional commit prefix. It is slightly long at 87 characters, but it is still clear.
Full details: Description check
Explanation
The description follows the template's main sections: Problem, Change, Scope and approval, and Verification. The Problem and Change sections clearly explain the bug, the retry and shutdown behavior, and the cleanup rules. Scope links issue #16238 and a triaged bug. Verification lists focused test results, typecheck, lint, and diff checks. It also discloses unverified runtime proof. The template asks for a conventional commit title, and the title meets that. The Scope section does not clearly show explicit maintainer approval of direction. It cites an automated triage flow instead, and the template asks for a maintainer approval comment or an explanation for a small fix. The Verification section is long and contains many links, but it includes concrete, reproducible results.

An interrupted Serve attempt that never succeeded must not turn off an
existing mapping on shutdown.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Fixed the interrupted-attempt cleanup finding in 47213bfe36. An attempt now records cleanup ownership only after successful setup. The existing hung-attempt regression assertion failed with one unwanted disable call before the fix and passes with zero afterward; all five focused tests pass. Current-head runtime transcript also shows zero disable calls and preservation of an existing fixture mapping. The runtime uses the real server with a labelled fake Tailscale CLI.

# ChildProcessSpawner
Service boundary for starting and controlling child processes.
`ChildProcessSpawner` is the service used by `ChildProcess` commands to start operating-system processes. A spawner turns a command description into a handle that can write to stdin, read stdout and stderr, wait for exit, kill the process, and manage whether the process keeps its parent alive. Platform backends implement this service, while most application code uses the higher level `ChildProcess` module.

## ChildProcessSpawner
`effect/process/ChildProcessSpawner` — unstable
Service that executes `ChildProcess.Command` values.
Key APIs: spawner.string(cmd), spawner.lines(cmd), spawner.spawn(cmd)
`string` collects stdout as a single string. `lines` collects stdout as `Array<string>`. `streamString` / `streamLines` are the incremental `Stream` forms, and `exitCode(cmd)` runs a command for its status alone; all five take `{ includeStderr }` except `exitCode`. `spawn` returns a scoped `ChildProcessHandle` with `stdout`, `stderr`, `all` (merged) as byte `Stream`s, and `exitCode` as an Effect waiting for process completion. Exit code is a branded `ExitCode`; compare with `ChildProcessSpawner.ExitCode(0)`.
Platform implementations: `NodeServices.layer` includes `NodeChildProcessSpawner.layer`; Bun uses `BunServices.layer`.
Termination waits, and only `forceKillAfter` bounds it. On Node and Bun, closing the scope around `spawn` (or calling `handle.kill(options?)`) signals the child's whole process group with `killSignal` (default `SIGTERM`), then waits for the leader to exit and up to one more second for descendants to disappear. Nothing escalates by default, so a child that ignores `SIGTERM` holds the scope — and therefore your shutdown — open until it exits by itself (probed: a `trap '' TERM; sleep 4` child delayed release by the remaining four seconds). Set `forceKillAfter` on the command (or pass it to `kill`) to send `SIGKILL` to the group at that deadline; the probe then released in 300 ms. The waits use native timers rather than the Effect `Clock`, so they elapse under `TestClock` without `adjust`. The Node and Bun spawners also clean up the group when the leader has already exited, successfully or by a signal, before the scope closes: a grandchild the command left running in the background (`sh -c 'sleep 47 & exit 0'` in the probe) is gone once the scope closes. That cleanup targets a numeric process-group ID, so if the group disappears and the operating system reuses its ID first, an unrelated …[truncated]</excerpt>

## Work package D — child processes and scopes
Effect 4 `ChildProcess.Command` is directly yieldable. Remove explicit process-start plumbing only when the caller needs a scoped process handle and no helper semantics are lost.
Keep `ChildProcessSpawner.string`, `lines`, `streamString`, `exitCode`, and related helpers when they own collection or exit behavior. Preserve explicit spawner capture where a returned service starts processes later; `effect-ai-claude-cli` is lifetime-sensitive and is not a textual cleanup.

Bound the Tailscale child-process cleanup.
When shutdown interrupts the retry in apps/server/src/tailscaleServe.ts, runTailscaleCommand closes the scoped child handle. Without forceKillAfter, a child that ignores SIGTERM can keep scope closure, and therefore server shutdown, waiting indefinitely. The fake test handle does not model this because its kill returns immediately.

Fixed the shutdown acknowledgement regression in eca52c3599. Setup plus success recording is uninterruptible again, matching the prior acquireRelease boundary; shutdown waits for successful acknowledgement and then disables Serve. The delayed-ack regression failed before the fix (expected one disable, received zero); all six tests now pass, including timeout without blindly disabling an unowned mapping. Server typecheck and targeted lint/format pass. Fresh actual-server A1/A2 runtime proof is explicitly unverified: VM startup stalls and terminal interruption prevented reaching the fixture milestones. The PR body records that gap and no longer presents the earlier rapid-shutdown capture as current evidence. The command timeout does not impose an absolute shutdown bound because subprocess cleanup can extend it.
The forceKillAfter limitation is real but predates this PR. runTailscaleCommand is unchanged; main already invokes it during uninterruptible Serve acquisition and release. The current setup/success boundary preserves that behavior. Forced subprocess termination is separate hardening, so this PR stays scoped to retrying failed setup and preserving cleanup ownership. The description explicitly disclaims an absolute shutdown-time bound.
Latest-head required checks passed at eca52c3599. The two resurfaced ownership/acknowledgement findings are fixed and resolved; the forceKillAfter suggestion has a written response documenting unchanged behavior on main. Macroscope still reports Needs Human Approval. Fresh actual-server A1/A2 proof remains unverified after VM interruptions; the description explicitly records the insufficient runtime audit and links the attempted capture. No merge performed.
Superseded by #26757, which folds in Serve retry with bounded backoff. You are credited with a Co-authored-by trailer. Closing this so the two don't conflict.

Problem
On Linux without Tailscale operator permission,
t3 serve --tailscale-servelogs a warning and reports ready, but remains loopback-only. Correcting the refusal does not publish the server until it is restarted. Desktop's Tailscale HTTPS setting uses the same setup path.Change

Verification
At eca52c3599, following the merge from main:
cd apps/server && ../../node_modules/.bin/vp test run src/tailscaleServe.test.ts: 6/6 pass. Covers recovery without restart, capped backoff, cleanup after success, shutdown during backoff, an unanswered attempt reaching its command timeout without disabling Serve, and shutdown while successful acknowledgement is pending.git diff --checkpasses.vp run typecheck: exit 0; no TypeScript errors (existing Effect suggestions reported).Server-only change; visual fixture matrix is empty.
cannot-verify server-runtime: fresh actual-server proof of delayed acknowledgement cleanup (A1) and timeout preservation of an existing mapping (A2) could not be completed on this VM.
Sanitized attempted runtime transcript · Controlled fake-CLI driver. These document failed capture attempts, not successful verification.
Historical real-server evidence retains its original SHAs: before 8d3b061121, after c9eb9eccde, and expanded audit at e3d87aa.
Historical fixture reproduction: driver. Unverified: real-daemon recovery on the current head (historical evidence above), actual Electron toggle, macOS login timing, Windows, and real logged-out stderr (the login fixture uses a fake CLI).
