Problem / Motivation

Follow-up answers can lose cited evidence after a reuse-only turn. Comments can disappear between retrieval and model context. Captions and visual observations can cite a broader window or the wrong frame, and answers can overstate what they inspected.

What changed

Each turn preserves the evidence it received and cited, with exact matching and collision rejection. Received video metadata carries forward too.

Comment reads return eight entries at a time, pinned to a saved page version. Search covers complete comment bodies and authors. Shortened passages retain attribution and disclose omitted content. Search excludes duplicate packet representations before the 20-result limit, uses SQLite FTS match spans for snippets, and reuses each parsed comment page within a search while checking deletion/version validity. Filtered saved reads distinguish match offsets from provider-page offsets.

Transcript findings use exact caption citations when their explicit support is unique, with window fallback for ambiguous passages. Visual findings belong to one frame; shared observations remain separate. Older ambiguous visual evidence receives a warning.

The finalizer receives durable records of this turn's retrieval, analysis, saved reads and inherited evidence. Publisher context and metadata budget protections remain intact alongside these citation rules.

The same storage now retains cited evidence across reuse-only turns.

Tests

Independent final-tree validation: library build, platform TypeScript and whitespace checks passed. Repository tests: 2,620 passed, 58 skipped. Separate auth/payment integration: 170 passed. Total: 2,790 passed, 58 skipped; suites ran sequentially. Additional independent probes covered deduplication, real SQLite token/accent matching, Unicode/control-character decoys, excerpt cutoffs, page caching, deletion/version safety, filtered paging and visual-provenance equivalence.

Regression coverage includes evidence lineage, comment paging, caption support, frame ownership, publisher context, metadata admission and answer-context budgets. Live provider/model and dashboard behavior have not been retested.

Compatibility

No additional model calls or budget increases. Per-run operation records initialize automatically in Durable Object storage. No D1 migration is required.

Independent review — 559e5dc3786435795d505f08082cfe86dbedfc08

Verdict: code review approved; merge readiness remains blocked by the Vercel authorization check. No material source-level findings remain in the approved scope. Keep the PR draft until the outstanding check is resolved or the maintainer explicitly determines it is not required.

Reviewed the full diff against 7e1429c942e2fd92a55202010158b62bcbb38fe3, including evidence lineage/legacy matching and deletion isolation, pinned comment pagination and match/author preservation, explicit caption support and fallback, structural frame ownership/shared-reference versioning, and durable operation provenance separate from billing. Earlier findings were corrected and retested. Writer/reviewer alignment is complete for this exact head.

Independent verification

Run in my own terminal against the final commit:

  • Full root npm test: 2,574 passed, 58 skipped, 0 failed.
  • Breakdown: library 105 passed/2 skipped; platform 2,000 passed/56 skipped; video catalog 11; user-account Workers 360; processor 49; frames 49.
  • Platform build/typecheck and git diff --check: passed; checkout clean.
  • Six additional regression tests are committed in platform/test/youtube-agent-evidence-review-regressions.test.ts. The eleven-restoration case is helper-level coverage, not an end-to-end model conversation.
  • GitHub Verify run 39093767281: succeeded for this head.

Remaining verification limits

  • Vercel currently reports failure because author deployment authorization is required; this is not a reproduced code/build defect. Preview deployment has not been validated.
  • Live Agent testing is paused at the owner's request. No claim that the recorded live issues are broadly resolved, and no deployed behavior verification.
  • Tests validate structural provenance/data flow, not whether every generated claim is entailed or every visual observation is correct. The skipped tests remain unverified.
  • Automatic visual reanalysis and dependent-memory correction are outside this PR's approved scope.

No merge or deployment performed.

Revised independent review — e1139b1

Verdict: CHANGES NEEDED. My earlier source-code approval for this head is withdrawn. Follow-up independent probes of the maintainer's feedback reproduce three material comment-search issues that the earlier suite did not catch. The Vercel authorization blocker also remains separate.

P2 — Duplicate comment representations consume the search limit

Packet indexing remains alongside whole-page indexing. Search limits rows before row-ID deduplication, but a packet excerpt and its saved-page equivalent have different IDs.
A fixture with 30 approximately 4 KB comments plus the ordinary first eight-comment packet produced 38 index rows. The 20 returned hits contained only 12 distinct comment positions: eight duplicates displaced other matches. The same baseline fixture had eight unique packet hits, without the new raw-page index. Please deduplicate by stable comment/page identity before limiting results, retaining full-body search coverage and citation provenance. Identical repeated packet saves are coalesced; this is not a claim of unlimited growth from identical calls.

Correction — Legacy packet truncation is not a new eight-entry regression

Kiro identified a baseline-probe dependency error: the extracted base projection helper imported the head's collection-kind helper. On the real base, youtube_comments is excluded from collection kinds and already uses the generic eight-excerpt cap. The initial twelve-to-eight regression claim is withdrawn. Legacy packets can still lack a convenient continuation in model context, but raw entries remain saved and search/version-pinned reads can recover them without mandatory upstream refetch. Treat any improvement here as a P3 compatibility/discoverability follow-up, not newly introduced data loss.

P2 — Matching passage can omit the actual search match

commentPassage uses lowercase substring matching while SQLite FTS uses whole tokens with accent folding. Real SQLite FTS5 fixtures reproduced:

  • art matches a late whole word, but the excerpt centers on the early substring in start and omits the real match.
  • cafe matches late café, but the excerpt falls back to the start and omits café.
Both returned 2,000-character passages without the actual match. Align passage selection with search tokenization/normalization, preserving original text and offsets.

P2 — Per-hit serial full-page hydration

Search loop awaits readSavedComment per hit. Each call rebuilds the asset inventory and reads/parses the full page; the pending-read map does not cache completed sequential reads.
After indexing, twenty hits on one page caused 20 serial blob reads, 20 JSON parses and 20 full asset-list scans in an instrumented storage fixture. Batch/reuse page and asset metadata within one search while preserving deletion/version and billing checks. This verifies amplification, not production latency, timeout, OOM or a measured billing increase.

P3 — Filtered saved-read continuation describes the wrong offset domain

Filtered read paginates matches, while coverage text describes provider-page positions and same-continuation paging. With 24 entries matching on even indices, the first eight matches return nextOffset=8. Following that through get_video_comments without pageVersion rejects; with the saved version it returns raw indices 8–15 rather than remaining matches 16,18,20,22. Calling the same saved-read method with the same query works.
Reachability limit: no current production caller passing a query to readEvidence or exposed read_session_evidence tool was found; current search uses readSavedComment. Treat as a latent contract inconsistency, not a demonstrated normal-agent failure.

Nonblocking cleanup

  • The deliver() doc comment now precedes recordOperation() in evidence-billing.ts:307–315.
  • Paged comment instructions repeat between collection coverage and COMMENT_PAGE_COVERAGE warning.
  • visual-provenance.ts:60–80 has near-copy finding/shared-finding loops. Refactoring must preserve their different combined-versus-shared semantics.

New verification and limits

Independent temporary probes ran against production-source methods/helpers; search probes used real SQLite FTS5 with mocked SDK/schema/storage transport, and paging probes used runtime Cloudflare shims with stubbed persistence/search lookup. All reproduction assertions passed. The initial legacy baseline comparison mixed dependency versions and was invalid; source inspection of the base helper AND its collection-kind dependency corrects that claim above. No new full-suite run, production Durable Object run, live model/provider test, implementation, push, merge or deploy. Working tree stayed clean. Earlier passing suite counts below remain valid historical results, but do not establish absence of these defects. Kiro agrees on the three P2 search findings and the latent P3 filtered-read mismatch, and correctly challenged the legacy regression claim. Overall verdict remains changes needed. Kiro's source assessment is separate from our executed probes.

Earlier validation record (retained for provenance; revised verdict above supersedes its conclusions)

Independent combined-head review — e1139b1

Verdict: CHANGES NEEDED for merge readiness: resolve the outstanding Vercel authorization check, or have the maintainer explicitly confirm that this preview check is not required. No source-code changes are requested by this review. Independent code review and Kiro's source review both find the combined changes sound, but I am withholding final merge approval while the check remains unresolved. Keep the PR draft for now.

Reviewed against merged PR212 base cade5df, including the previous 559e5dc changes, full current diff, surrounding code, prior review and all four conflict resolutions.

Combined behavior verified

  • Frame and storyboard analysts preserve both atomic/single-frame versus shared findings and untrusted publisher-context guidance, schemas and validation.
  • Transcript tool wording retains honest focused-analysis disclosure and explicitly allows the missing-metadata lookup.
  • Exact caption-support matching, numerical support and ambiguous window fallback remain intact. Publisher names do not incorrectly narrow caption citations.
  • Version-pinned eight-comment paging, tail-match/author preservation, exact cited-evidence lineage and ambiguous-version rejection remain intact.
  • Per-run operation disclosure remains durable and separate from billing.
  • PR212's metadata reservation and finalizer description deduplication survive the merge; all four conflict files preserve both branches' behavior and tests. Kiro independently agrees on this exact combined head and found no new material issue.

Fresh independent checks in my own terminal

  • Full root suite: 2,587 passed, 58 skipped (library105, platform2011, catalog11, runtime/account362, processor49, frames49).
  • Separate auth/payment integration: 170 passed. Combined unique total: 2,757 passed, 58 skipped, zero failures.
  • Additional focused rerun: 52 passed; not added to the unique total.
  • Library build, platform TypeScript check and git diff --check passed.
  • Sequential four-plus-four research retains all eight transcripts/analyses for successful and failed metadata lookups.
  • Original long-description projection probe retains all eight analyses with no missing inputs (6,617-character payload including the combined disclosure fields); 20,000-character-description and citation-preservation regressions also pass.
  • Verify CI 39105824018 completed successfully for this head.

Outstanding external check

Vercel reports failure because this commit needs author deployment authorization. This is not a reproduced build/code defect. Please resolve it through the PR's Vercel check, or explicitly decide that preview deployment is not required. No authorization, merge or deployment was performed by this review.

Verification limits

No fresh live provider/model/dashboard test or production deployment verification. Kiro's review was source-only; executed counts above come from independent local checks. Full root production build, local browser E2E and Docker image build were not independently run; GitHub CI is separately reported. Skipped tests remain unverified. Structural provenance checks do not establish semantic truth of every generated claim. Previously noted nonblocking metadata warning/non-transcript-reservation and unexecuted storyboard-cancellation caveats remain; automatic visual reanalysis/dependent-memory correction remain outside scope.

Revised review — 86d6748 (October 19, 2026)

Verdict: CHANGES NEEDED. Earlier source-code approval for this head is withdrawn. Three follow-up findings are independently reproduced. Historical passing tests below remain valid, but did not cover these cases.

P2 — Duplicate suppression performs a correlated full packet scan

platform/src/agents/runtime/session-search.ts:171–173 compares owner='packet:' || p.packet_id, preventing the packet primary-key lookup. The exact production query in local Cloudflare workerd shows a correlated SCAN p. With 20 asset-owned matches and 100 / 1,000 / 10,000 saved packets, it reads 2,019 / 20,019 / 200,019 rows. A packet-owner guard plus p.packet_id=substr(session_context_fts.owner,8) uses the primary-key index and reads 19 in this fixture, preserving duplicate/legacy fallback semantics. This affects transcript matches as well as comments; LIMIT 20 does not bound the preceding correlated work. Introduced in PR211's duplicate filter. Cloudflare uses SQL cursor row-read counts for billing (documentation); these are local measurements, not a measured production charge.

P2 — Unrelated deletions discard evidence-search results

session-evidence.ts:573–582 reloads an unchanged page when the cached generation is stale, then rejects it because the cache generation is never refreshed. Deleting an unrelated memory (even a nonexistent ID) or another asset changes the global generation (:850–862). session-search.ts:211 does not isolate this recognized invalidation race. Production-method probes show the requested page remains present, yet the whole evidence search is lost; immediate retry succeeds. The actual SDK search_context wrapper returns an Error string, so this is not proof that the entire Agent run fails.

Nuance: the lower-level readStored generation fence predates the cache, and deletion during awaited hydration also failed on e1139b1. The additional stale-cache failure after a successful fresh reload is new. Refresh/invalidate the per-search cache and safely retry unchanged versions or skip deleted ones; preserve deletion, final availability and resurrection fences. Removing one generation comparison alone does not fix every race. Controls verify deleted pages/full clears remain withheld. Existing tests cover deletion after a completed search, not these interleavings.

P2 — Redundant persisted comment-search snapshots amplify storage and scans

session-evidence.ts:585–587 keys each persisted search packet by raw first-match start/end, while emitted citation/passage identity uses the normalized passage start (:605–609). In local workerd, 20 unchanged short comments searched with alpha / alpha / alpha beta / beta / gamma / delta / VIEWER / viewer leave 20 / 20 / 20 / 40 / 60 / 80 / 100 / 100 packets, but only 20 distinct citation IDs. Identical keys replace existing rows, so “every distinct query creates a new row” is too broad; differing match spans can create redundant copies of identical returned evidence. Key by emitted passage identity while preserving genuinely distinct passages and historical citation resolution. Do not collapse all passages to one row per page/comment.

These rows amplify the new correlated scan above. Separately, PR213's current inspectionRecords path calls evidence() before filtering visual packets, loading/parsing the whole packet table; that downstream interaction is being reviewed separately.

Verification and limits

Two local workerd production-method tests passed for SQL rowsRead/query plans and packet growth. Deletion probes used production store/search methods, real SQLite FTS, production schemas and the pinned SDK wrapper with controlled R2-adapter interleaving; same-page deletion and clear controls passed. A separate deletion workerd harness could not start due to Miniflare configuration incompatibility; no live DO scheduling, production bill or live Agent failure was measured. No implementation, push, merge, deployment or full-suite rerun. Feedback relayed to Kiro for source/design reconciliation.

Historical validation record (the revised verdict above supersedes approval/readiness conclusions)

Independent re-review — 86d6748

Verdict: CHANGES NEEDED for merge readiness only: Vercel requires author deployment authorization. Verify CI has passed. The three reproduced P2 code findings are fixed and independently retested. Kiro and reviewer agree on the final code; no further code changes are requested by this review. Keep draft until outstanding checks are resolved or the maintainer explicitly determines the Vercel preview is not required.

Verified the pushed SHA by PR API and Git fetch. Its tree 7f0eeaa6d1111fd03ba3136aeafa911dd418fad0 exactly matches the independently tested local tree. Reviewed against previous head e1139b1; PR base remains cade5df.

Findings retested

  • Duplicate comment hits: saved-page representation is selected before the 20-result limit; the fixture now yields 20 distinct comments rather than 12. Legacy packet-only search and existing packet citation resolution remain available. Relevant: session-search.ts:160–187.
  • Incorrect snippets: SQLite FTS supplies actual match spans. Reproductions covering art/start, accents, ligatures, fullwidth/circled characters, astral letters, controls, author-only matches and whitespace pass. Per-query markers are collision-checked and must round-trip to source text. Full matched tokens survive excerpt-cutoff boundaries. Relevant: search-highlight.ts, get-video-comments.ts:174–207.
  • Repeated hydration: twenty same-page hits now use one page read/parse and one asset inventory scan. Independent two-page/version, refresh, reopen and deletion-during-hydration probes pass. Relevant: session-evidence.ts:565–590.
  • Latent filtered paging contract: matching counts/offsets and same-query instructions are separated from raw provider-page positions; normal saved reads retain a pinned version. Twenty independent paging/recovery assertions pass.
  • Cleanup: delivery documentation moved, duplicate paging prose removed, and visual loops consolidated. 512 independent before/after visual-output/error comparisons pass.
  • Legacy correction retained: the prior twelve-to-eight regression claim was invalid due to mixed baseline dependencies. A coherent baseline and head both show eight. This is not a new cap regression or permanent comment loss.

Final independent validation

  • Root suite: 2,620 passed, 58 skipped (library105, platform2021, catalog11, account/runtime385, processor49, frames49).
  • Separate auth/payment integration: 170 passed. Total 2,790 passed, 58 skipped.
  • Library build, platform TypeScript and whitespace checks passed.
  • Suites ran sequentially. Additional probes are not added to suite totals; they use production methods with real SQLite FTS5 and mocked SDK/storage plumbing. Repository Workers integration is included in the root suite.
  • Earlier draft patches failed an oversized test fixture and independent Unicode/boundary probes. These were corrected and the full final-tree validation rerun; the passing counts above are for the final tree.

Outstanding checks and limits

Verify CI 39134675231 succeeded: documentation, library/skill bundle, web, both dashboard E2E groups, platform checks/integration and auth E2E all passed. Vercel reports author deployment authorization failure, not a reproduced code/build defect. No settings or credentials changed, no merge or deploy.

No fresh live provider/model/dashboard or production performance test. The measured storage-call reduction is a local instrumented result, not a production latency guarantee. Full root web build/browser E2E/Docker build were not independently run. The pre-existing saved-read provider-continuation disclosure caveat is outside these changes. PR213 is stacked on the earlier PR211 base and will need its actual resulting base/head checked when retargeted.

… reuse saved search rows

The duplicate filter compared an expression over packet_id, which forced a full scan of session_packets for every match. It now compares the bare key.

A comment search no longer fails when a different asset or memory is deleted mid-search; only a deleted page's own hits are skipped.

A search hit's saved packet is named after its passage, so repeated searches replace one row instead of adding another.

Verdict: CHANGES NEEDED for 9ba6ba8.

P2 — An unrelated deletion during comment-page hydration still silently drops the current search hit.
In platform/src/agents/runtime/session-evidence.ts:579-583, a null page clears the cache and returns no packet, relying on a later hit to retry. loadCommentPage at :593-595 maps a generation-invalidated read to undefined. If an unrelated memory is deleted while the awaited blob read is in flight, the requested page still exists, but this hit is discarded rather than safely retried.

Independent tests using actual local workerd, SQLite, R2 and the production SessionEvidenceStore reproduced:

  • One matching comment: first search returns 0, immediate retry returns 1.
  • Four matching comments on one page: first search returns 3, retry returns 4.
  • The real SDK search_context tool returns ordinary successful JSON with packets:[] in the one-match case, with no omission warning. This can make the model conclude that matching saved evidence does not exist.

The new repository test deletes before hydration starts; it does not cover this awaited-read interleaving. Retry the affected hit when the same immutable version remains valid, or explicitly surface incomplete results, preserving same-page deletion/full-clear/resurrection fences. Do not simply remove the lower-level generation check. Same-page deletion and full-clear controls pass; cached-page reuse after unrelated deletion also passes. This is an incomplete fix of the earlier invalidation finding, now exposed as silent omission rather than an error for the whole search. No live user answer was induced.

Prior findings resolved in this head:

  • session-search.ts: packet duplicate suppression now uses the packet_id primary-key lookup. Independent workerd EXPLAIN confirms the indexed lookup; a mixed 5,200-row SQLite fixture preserves top-20 results.
  • Search packet identity is based on the actual excerpt, not the raw query span. Independent probes confirm identical passages replace one saved row across queries/reopening, distinct passages retain distinct immutable citation IDs/text, and legacy provider/search citations still resolve.

Separate merge-readiness blockers:

  • A read-only merge-tree comparison with actual main c2727e2 reproduces conflicts in tool-context.ts, analyze-video-frames.ts, analyze-video-storyboard.ts, agent-runtime-do.integration.test.ts and session-evidence.integration.test.ts. The standalone head has not been validated combined with merged PR217.
  • Vercel reports Authorization required to deploy for this SHA. No GitHub Verify workflow run was returned for this head. This is not a demonstrated source build failure; no settings or author permissions were changed.

Verification: 2,021 platform unit tests passed / 55 skipped; 69 evidence Workers integration tests passed; 59 focused fidelity/lineage/comment tests passed (overlapping rerun, not additional unique total). Independent deletion probes: 2 all-hits assertions failed, 3 safety/reuse controls passed. Two additional passage/legacy-identity probes passed. Final TypeScript and full-PR whitespace checks passed. A temporary probe-only type error was corrected before the final passing typecheck; production files were not edited.

Limits: no fresh live provider/model/dashboard tests, no full root/auth rerun, no integrated-main test. Kiro consultation for this head is currently blocked on sign-in; no new writer/reviewer agreement is claimed. No implementation, push, merge or deployment performed.

A deletion elsewhere in the session fences an in-flight read, which returns nothing even though the page still exists. That hit is now read again, up to three times, instead of being dropped from the results. If the session keeps changing, the search fails with a retry message rather than returning an empty result. Deleting the page itself still returns nothing for it.
Conflicts: main moved frame and storyboard packet construction into shared analysis helpers that retrieval-time analysis also uses. The single-frame provenance from this branch now lives in those helpers. Both sides' new tests are kept; one test follows main's delete() return value.
Since retrieval tools can analyze content while saving it, a get_video_frames, get_video_storyboard or get_video_transcript packet can hold analyst findings. Its delivery label now follows the analysis artifact it carries, so the finalizer no longer calls those observations plain retrieval data.

Independent re-review — 48c27b8

Verdict: CHANGES NEEDED for merge readiness only. No material source-code findings remain from this review. Resolve the Vercel author-authorization check, or have the maintainer explicitly determine it is not required. Keep draft pending writer/reviewer alignment; current Kiro consultation is blocked on sign-in.

Previous P2 fixed

platform/src/agents/runtime/session-evidence.ts:586–617 retries the same interrupted comment hit when its immutable page still exists. The original independent local workerd/SQLite/R2 probes now return all one- and four-match results, including through the actual SDK search_context wrapper. Same-page deletion and full-clear controls still withhold deleted evidence, and cached reuse survives unrelated deletion. Repeated interruptions produce an explicit retry error after three attempts rather than a misleading successful empty result; repository integration coverage passes. This closes the last reported comment-search omission finding.

Integration and intended behavior

  • Main c2727e2f53d568e8c6b7fad8cdccb35c32c0dc25 is now an ancestor. Reviewed all five conflict resolutions; the previous merge conflicts are resolved and the combined tree is tested.
  • analyze-video-frames.ts:85–92 and analyze-video-storyboard.ts:103–111 retain atomic frame findings and separate shared observations inside common helpers serving both saved and retrieval-time analysis.
  • model-evidence.ts:539–558,627–634 classifies combined retrieval analysis by its artifacts. Two independent combined-tool probes confirm actual frame/storyboard packets preserve provenance through finalizer projection, while saved-read/inherited packets are not relabelled as newly performed analysis.
  • Earlier indexed duplicate suppression and stable comment-passage identity remain intact.

Independent checks

  • Full root suite: 2,729 passed, 61 skipped (library105/2 skipped; platform2100/59 skipped; catalog11; Workers415; processor49; frames49).
  • Separate auth/payment integration: 170 passed. Combined standard total: 2,899 passed, 61 skipped.
  • Seven additional reviewer probes passed: five deletion/reuse and two combined visual-provenance/label checks. Not included in standard totals.
  • Library build, platform TypeScript and full-PR whitespace checks passed; checkout clean after removing reviewer-created probes.
  • Initial root attempt mistakenly discovered a temporary workerd probe under the default Node configuration, causing a setup/import failure. Corrected its placement and reran the complete standard suite successfully; this was not a production-source test failure.
  • Verify CI39421252744 succeeded for this exact head.

Remaining limits

At 12:25 UTC on October 20, Vercel still reports Authorization required to deploy. This is not a reproduced code/build defect; no author permissions or settings were changed. Kiro is signed out, so no fresh agreement or ready-for-review action is claimed. No fresh live provider/model/dashboard, production-performance, local browser E2E, web-build or Docker-build verification. Passing structural tests do not prove semantic accuracy of every generated observation; skipped cases remain unverified. No implementation, push, merge or deployment performed.
