• McpToolAdapter::with_prefix and from_client_with_prefix now name the tool <prefix>__<tool> (fix(mcp): with_prefix names the tool <prefix>__<tool> #323). Before, the prefix was stored but never used, so two servers offering the same tool name, or a server tool named like a built-in, gave the agent duplicate names, which providers reject.
  • The server is still called with its own tool name.
  • The MCP guide shows how to prefix a server's tools; the with_mcp_server_* helpers add no prefix.
