) Decision record for the documentation truth-up epic (#70): fixes the information architecture (one home per knowledge type — README / role_and_seams / ADRs / CICs) and the ADR-supersession policy (write a new superseding ADR, don't rewrite Accepted ones; ADR-001 -> Superseded by ADR-012, with ADR-002 extended in place).
