TIN never inserts empty documents into the index, which means they don't count toward the number of documents or average doc length for BM25 calculations. Lead now matches that behavior.

This backports some tinql and boldi-vigna bugfixes from TIN v1.0.6.
It also fixes a discrepancy between TIN BM25 scoring (which ignores empty documents) and Lead BM25 scoring (which didn't, but now does).
