fix(server): explain provider updates Windows blocks from starting - #28858
On Windows, when Smart App Control or another application control policy blocks the provider updater (for example vite-plus\bin\vp.exe for a Vite+-installed OpenCode), the provider update fails and the only reason shown is a bare spawn UNKNOWN. Node throws that error synchronously from ChildProcess.spawn. Effect's NodeChildProcessSpawner therefore turns it into a defect, which skips the runner's Effect.mapError. recordFailedUpdate then stores the squashed message spawn UNKNOWN as the update-failed reason.
Reproduced on Linux through the same Node branch: the real Effect spawner with an argv Node rejects synchronously (E2BIG) gives a cause with hasFails: false, hasDies: true, squashed to "spawn E2BIG". The red test below models the Windows UNKNOWN case.
Change
collectCommandResult in apps/server/src/provider/providerMaintenanceRunner.ts now adds an Effect.catchDefect after the existing mapError on spawner.spawn(...), the same pattern htmlRender/headlessChrome.ts uses. A synchronous spawn failure now becomes the existing ProviderMaintenanceCommandError (Failed to run update command <command>: <reason>). On win32, a spawn error with code === "UNKNOWN" gets this reason: Windows could not start it (spawn UNKNOWN). Smart App Control or an application control policy may be blocking this executable. Any other synchronous spawn failure reports its error code (spawn E2BIG), not the raw error text. The catch wraps only the spawn call, so interruption, timeouts and stream errors behave as before. No contract, client or persisted-state changes.
Triaged via the automated issue flow. This covers the issue's first suggestion (say why the launch failed); the npm fallback it also suggests is out of scope here.
Verification
vp test run apps/server/src/provider/providerMaintenanceRunner.test.ts: 31/31 pass. The new test explains a Windows-blocked updater launch on win32 fails on main with AssertionError: expected 'spawn UNKNOWN' to not equal 'spawn UNKNOWN'.
vp run check:changed: format, lint, knip, graphql, typecheck (3 packages), tests (2 files) pass.
No captures: only the server's update-failed message text changes, and clients render it as before. The Windows trigger cannot be produced in a client on Linux.
cannot-verify windows: no Windows machine with Smart App Control was available. The UNKNOWN path is covered by the unit test, and the Node mechanism was confirmed on Linux with E2BIG.

These tests reproduce #28843: when Windows blocks the provider updater
executable, Node throws `spawn UNKNOWN` synchronously and Effect's
spawner surfaces it as a defect, so the update fails with the bare
"spawn UNKNOWN" message instead of explaining the launch was blocked.

Node throws `spawn UNKNOWN` and similar errors synchronously, which
Effect's spawner surfaces as a defect, so the update failed with the
bare "spawn UNKNOWN" message. Catch the spawn defect beside the existing
mapError and, on Windows, explain that Smart App Control or an
application control policy may be blocking the executable.

Walkthrough
The provider maintenance runner now converts synchronous process-spawn defects into ProviderMaintenanceCommandError. On Windows, an UNKNOWN defect receives Smart App Control or application-policy guidance. A test checks the resulting update failure message.
The runner reports Windows UNKNOWN defects as possible application-control blocks. Other coded defects include the spawn code, and uncoded defects report that the process could not be started. The runner retains the original cause. The test checks the Windows failure message.
No actionable merge-blocking risk remains in the provider update error-handling change.

Explanation
Issue #28843 is the only active direct link. The PR addresses its first coding objective. runProviderMaintenanceCommandWithSpawner adds Effect.catchDefect around the spawn operation. On win32 with error code UNKNOWN, it reports a Windows application-control warning instead of bare spawn UNKNOWN. The added test verifies the failed update state and the warning text, including vp and Smart App Control. The npm or provider-command fallback is a suggested improvement, and the PR explicitly excludes it.

Explanation
The diff changes only the provider maintenance runner and its test. The production change converts synchronous spawn defects into the existing ProviderMaintenanceCommandError. The test covers the linked issue behavior. No unrelated contract, client, persisted-state, resolver, or fallback changes appear in the diff.

Explanation
The pull request description includes all required sections. It explains the Windows spawn failure and expected behavior, describes the scoped implementation, links the triaged issue, records verification results, and states the Windows verification limitation. It also identifies the models and harness used.
