fix(server): preserve approval answers through provider acknowledgements - #25033
Pi, OpenCode and Muse acknowledgements can overwrite an approval decision or question answer that T3 already recorded. Muse also marks declined approvals completed and can reopen answered approvals during stage updates.
Change
Keep the orchestrator's response authoritative and use acknowledgements for adapter cleanup. Preserve Muse decline status, retain automatic/native decision handling, and give a genuinely new Muse requirement its own request after acknowledgement. Publish cards before requests so immediate answers settle the card too.
Reuse @saphid's implementation and replay coverage in main's split suites. Preserve current main's hidden automatic child approvals, including stale-decision failures.
Verification
Final CI passed on e44aa96, after merging github/main. Both independent final reviewers approve this exact head with no remaining source findings. The Pi ordering finding is addressed and its review thread resolved.
Reproduced six failing replay assertions on current main before changing production code.
blacksmith-check: vp run check:changed passed formatting, lint, knip, GraphQL generation validation, seven affected package typechecks and all 202 tests across six replay/adapter files.
Replay covers approvals, declines, questions, acknowledgement ordering, turn cleanup, Muse stages and automatic child failures through the real adapters, orchestrator, effect worker and persisted projections.
Unverified: live Pi/OpenCode/Muse account and client interaction, reconnect, desktop, mobile and remote/tunnel flows. The dev server and real browser run, but Pi reports no usable authenticated models and Muse is unavailable. Before/after UI screenshots and interaction video remain outstanding; replay results do not establish live UI verification.

Walkthrough
Provider adapters retain submitted answers and decisions during native delivery or settlement. They update request and session projections without replacing persisted responses. Adapter and replay tests cover Muse, OpenCode, and Pi request outcomes.

Summary
Muse response ownership and approval lifecycle packages/provider-muse/src/server/adapter.ts, packages/provider-muse/src/server/adapter.test.ts
Muse records response ownership and resolution time, tracks approval round trips, and updates request projections during native and orchestrator resolution. Tests cover acknowledgements, staged requirements, and session state.
OpenCode and Pi settlement updates packages/provider-opencode/src/server/adapter.ts, packages/provider-pi/src/server/adapter.ts, packages/provider-pi/src/server/adapter.test.ts
OpenCode stores submitted responses, recalculates waiting status, and includes stored responses in native settlement. Pi emits node and turn-item updates before runtime requests and no longer emits resolved-state updates after responses.
Replay outcomes and projection assertions apps/server/src/orchestration-v2/testkit/*ReplayFixtures.integration.test.ts, apps/server/src/orchestration-v2/testkit/fixtures/*/output.ts
Replay tests cover Muse questions and approval, OpenCode child-approval decline, and Pi accept, decline, and text-answer requests. Fixture assertions check response values and associated node and turn-item statuses.

Explanation
The title "fix(server): preserve approval answers through provider acknowledgements" matches the main change. The summary shows the adapters for Pi, OpenCode, and Muse now keep the orchestrator's recorded approval decisions and question answers when provider acknowledgements arrive. The title is concise, uses a conventional commit prefix, and names the primary fix. It does not mention the Muse decline status or the publish-order change, which are secondary.

Macroscope's review found this PR requires human approval — This PR changes approval and question settlement, event ordering, provider-session status, and persisted node/card state across the Muse, OpenCode, and Pi production adapters. Although replay and adapter tests provide broad coverage, the cross-provider lifecycle and acknowledgement changes warrant human review.

reproduced on github/main (48a292364ab37574c67fb377b5227e5d281d9203): in supervised pi, request printf approval-verification, then click decline. after acknowledgement, the saved decision was missing and the node/card were completed.
repeated the same prompt and decline on the pr: decision remained decline; node/card remained cancelled. the agent acknowledged the decline without running the command.
approved the next pi command: decision remained accept, node/card completed, and the client displayed approval-verification.
answered a real opencode question with Keep tests focused: the provider received it, and both the saved request and question card retained the answer. an opencode command decline also retained decline with cancelled node/card.
