fix(server): report a created PR as created when its lookup fails - #23242
"Create PR" (and "Commit, push & create PR") can report failure for a PR it already opened. runPrStep calls createChangeRequest, which returns nothing, and then calls findOpenPr to read the new PR's URL and number. When that second call fails (a 502, a rate limit, a CLI timeout), the error goes up through runStackedAction as action_failed in the pr phase. The user sees "Action failed" while the PR exists on the host, and the Git panel still offers "Create PR".
To reproduce: make the provider's PR list call fail once, right after a successful create, then click Create PR.
Change
runPrStep now catches an error from the lookup that runs after the create, logs a warning, and treats it like an empty lookup. That path already exists: when the lookup succeeds but finds nothing, runPrStep returns status: "created" with the base branch, head branch and generated title, and no URL or number. The fix only routes the error case into it. The lookup before the create is unchanged, so an earlier failure still stops the action before anything is created.
One focused test in GitManager.test.ts wraps the existing fake GitHub provider so listChangeRequests fails once createChangeRequest has run. It asserts the action resolves with status: "created", the head branch, and no URL.
Scope and approval
There is no issue. This is a small fix of a clear bug: a step that already succeeded is reported as failed, and the code already has a result shape for "created, metadata unknown". Server only, 10 production lines in one file.
Verification
Focused test, with main's GitManager.ts and then this branch (pnpm exec vp test run src/git/GitManager.test.ts --reporter=verbose -t "<name>" from apps/server). On main it fails with SourceControlProviderError: ... listChangeRequests: GitHub API rate limit exceeded. On this branch it passes.
Whole GitManager.test.ts on this branch: 105/105 pass, with the host Git config and with GIT_CONFIG_GLOBAL=/dev/null.
tsc-rs --noEmit in apps/server: exit 0. vp lint and vp fmt --check on both changed files: clean.
Real app, Linux, production builds of main and this branch (vp build web, vp pack server), each with fresh state. The repo's GitHub remote points at a GitHub Enterprise host on localhost. A small HTTPS stub answers its REST and GraphQL API. After it accepts POST /pulls it answers the next PR lookup with HTTP 502, then recovers. PR text came from Pi with a local stub model. No real GitHub host or token was involved.
Main: the stub records the create (201) and then the failed lookup (502). The app shows "Action failed: Source control provider github failed in listChangeRequests: GitHub returned HTTP 502." and the panel still offers Create PR.
This branch, same stub sequence: the server logs the warning and the Git panel shows "Created PR" with the generated title.
Not checked:
Real GitHub, GitLab, Azure DevOps, Bitbucket or Forgejo hosts. The app run used a local GitHub Enterprise stub, and the test uses the fake GitHub provider.
Desktop and mobile clients. Windows and macOS.
With the fix the result has no URL or number when the lookup fails, so the toast has no PR number and linkCreatedPullRequest skips it. I did not check whether refreshPushedPullRequests or the next status refresh links the PR to the thread, or how soon.
Prior PRs
None for this bug. #20752 by @THEjacob1000 (open) moves runPrStep into sourceControl/ChangeRequestStep.ts without changing this lookup. Whichever lands second needs to carry the same few lines over. #22943 by @juliusmarminge (open) changes the results runPrStep returns right after this lookup. The two branches merge without conflicts.
createChangeRequest has already opened the PR when runPrStep asks the
provider for its metadata. A failed lookup now logs a warning and returns
the existing created-without-URL result instead of failing the action.

Walkthrough
After pull request creation, a failed follow-up lookup no longer fails the action. The action returns a created result without pull request metadata. A regression test covers a provider rate-limit error.
runPrStep logs lookup failures and continues without pull request metadata. A regression test checks that a rate-limit error still results in a created pull request without a URL.
The create action now reports success when the follow-up lookup fails. No identified issue prevents merging after normal checks.

Macroscope's review found this PR approvable — This is a focused server bug fix that preserves successful PR creation when the subsequent metadata lookup fails, while logging the lookup failure and returning the existing metadata-less created result. The added test covers the failure-after-creation path, and no defaults, schemas, infrastructure, or diagnostic suppressions are changed.

verified the reachable paths on linux at aa80f96be5ae0d054b06a672a8ba1b28cc2d1248. this is partial verification, not proof of the changed failure path.
installed and ran github/main (32ecd71726) in /home/user/main, then stopped it before running the exact pr head with isolated state. one dev server ran at a time.
drove the web app with the repo's playwright and /usr/bin/google-chrome. added a project through the app pointing at a real clone of the contributor branch. real codex gpt-6.1-sol turns completed on main and the pr head; the pr turn reported the exact head and clean checkout.
the real github provider discovered https://git.cafe/pingdotgg/t3code/pulls/23242 in the git panel. an authenticated websocket git.runStackedAction request with action: "create_pr" returned action_finished, pr.status: "opened_existing", number 23242, and its real github url. the ui correctly omits create pr when that pr exists.
a second request against the detached exact-head checkout returned action_failed: Cannot push from detached HEAD.
blacksmith ran the existing post-create-lookup-failure and existing-pr tests: 2 passed, 103 skipped. these tests use the existing fake provider and do not establish real-host failure recovery. ci currently reports 28 successful checks.
the original bug was not reproduced on main, and successful real pr creation followed by a metadata lookup failure was not exercised. https://git.cafe/pingdotgg/t3code/pulls/23242 already exists, so it returns before the changed code. reaching that code requires another real pr, which conflicts with this task's hard rule to touch no other pr. no additional pr was created and no mock or stub was substituted.
also unverified: recovery of metadata and thread linking after the failed lookup; the combined commit/push/create flow; other source control hosts; other agent providers; desktop and native ios/android; macos/windows; remote/relay/tunnel connections. no source changes or pushes were made.
takeover update
merged current main without conflicts and updated the regression test to use main's renamed provider helper. the production behavior is unchanged. the review finding about raw error logging is addressed: the warning now records only provider kind and error tag. final head: 46fdd68ea57ba62aede9ddf2e4736ca479c7d890.
blacksmith vp run check:changed passed format, lint, knip, graphql, affected typechecks, and 116 tests across the git manager and agent context suites. the stale helper failed first, then passed after the one-line fix. two independent scoped reviews approved this head. all 29 ci checks are green on this head, github reports no conflicts, and the review thread is resolved. the real-host failure scenario remains unverified for the reason above.
