Applies the lesnik512/planning-convention APPLY.md 1.x->2.0.0 update: swap index.py + templates (drop plan.md), flatten the one existing change folder to a flat change file, merge the 2.0.0 convention prose into planning/README.md, retire "bundle" wording in CLAUDE.md/Justfile, and record .convention-version=2.0.0.
