Apply the lesnik512/planning-convention APPLY.md 1.x->2.0.0 update: swap index.py and templates (dropping plan.md; glossary/decision/release templates already matched canonical), merge the 2.0.0 flat-change-file prose into planning/README.md, and align CLAUDE.md's "bundle" wording to "change file".
