Applies the lesnik512/planning-convention APPLY.md update 1.1.0 -> 2.0.0: swap index.py + templates (drop plan.md template), flatten the single change folder to a flat change file (dropping the committed plan.md), merge the 2.0.0 convention prose into planning/README.md, retire "bundle" wording in CLAUDE.md, and record .convention-version=2.0.0.
