Fixes #1484.

A secret-less MCP server has no Token Manager record. The queue processor now uses the API's masked env/headers metadata to skip secret resolution for those servers, so a successful run no longer logs a misleading hydration warning. Servers with stored secrets are still resolved, and resolution failures still warn.

Updated the MCP lifecycle documentation and added dispatch tests covering secret-less, successful hydration, and resolution-failure paths.

Validation: pnpm exec vitest run packages/shared/src/queue/queue-processor.test.ts (34 passed); pnpm --filter shared exec tsc --noEmit; git diff --check.
