@hmorris3293 I made appropriate edits and added a missing email address field and a reusable component on a custom domain as when checking the code for the app https://github.com/akamai-compute-marketplace/marketplace-apps/pull/432/changes#diff-80f5b813f2cc96d3d939ca9d3535e73713ba35fdd51a36d4b259150577b483a6R32, the custom domain-related fields were included there.
