Keep default_version consistent with a stored version.

The model accepts and commits Prompt(id="pmpt_001", default_version=1) without a (pmpt_001, 1) row; the new tests do exactly that. A lookup through the default-version pointer can therefore find no template. Create the initial version and set the pointer as one atomic operation, then enforce the (id, default_version) relationship in the persistence design. (docs.sqlalchemy.org)

Review comment at @src/models/database/prompts.py at line 21:
Update the Prompt persistence design so creating a prompt atomically creates its
initial version and sets default_version to that version; enforce that the
pointer references a version belonging to the same prompt, including through a
database constraint on the prompt ID and default_version relationship.

@Jazzcort Understood. This can remain an operation-layer follow-up rather than require a model change in this PR. The consistency check remains unverified until that work is implemented.

Would you like a GitHub issue to track the follow-up?

Enforce the stated immutability of prompt versions.

PromptVersion describes immutable rows, but an ordinary ORM update can change a persisted row’s template or variables and commit it under the same (prompt_id, version). That changes the content associated with an existing version. Reject updates to version content, or make immutability a documented and enforced write-layer contract. (docs.sqlalchemy.org)

Review comment at @src/models/database/prompts.py at line 33:
Enforce the immutability documented by PromptVersion: prevent persisted rows’
template or variables from being changed under the same prompt_id and version,
using the existing write layer or ORM update handling. Keep creation of new
version rows unchanged.

Add a module docstring to unblock pydocstyle.

Review comment at @src/prompts/exceptions.py at line 1:
Add a brief module docstring at the start of exceptions.py, before the
PromptError class, to satisfy pydocstyle’s D100 check.
