Add opt-in header X-LCS-Shield-After-Last-Refusal on /v1/responses so client-owned-context sessions (store: false) can continue after a shield refusal without re-moderating the full resent history.
When set to true, shields run only on input items after the last prior refusal (modr-…). Default remains full-input moderation (no breaking change). Refusal messages now carry id=modr-… so clients can keep them in history for windowing.

Checklist before requesting a review
I have performed a self-review of my code.
PR has passed all pre-merge test jobs.
If it is a core feature, I have added thorough tests.
Testing
Enable shields and call POST /v1/responses with full history including a prior refusal item (id: modr-…) plus a clean new user turn.
Without the header: moderation still sees the full history (can re-block).
With X-LCS-Shield-After-Last-Refusal: true: only the suffix after the last modr-… is moderated; a clean follow-up should proceed.
Unit coverage: TestGetShieldModerationInput, endpoint header windowing tests in test_responses.py.
Summary by CodeRabbit
New Features
Added the optional X-LCS-Shield-After-Last-Refusal header for /v1/responses. When set to true, moderation checks input after the most recent refusal; if there is no prior refusal, it checks the full input. Without the header, full-input moderation remains unchanged.
Bug Fixes
Refusal responses now use the corresponding moderation ID as their response ID.
Documentation
Updated the shields guide and developer documentation with the header’s behavior and usage.
The Responses API can limit shield moderation to input items after the last moderation-ID response message. The new request header controls this behavior. Refusal response messages now include their moderation ID.

Summary
Moderation IDs and input selection src/models/common/moderation.py, src/utils/responses.py, tests/unit/utils/test_responses.py, tests/unit/utils/test_shields.py, tests/unit/pydantic_ai_lightspeed/capabilities/question_validity/test_capability.py
Refusal response messages now include the moderation ID. New utility functions select input after the last message with a moderation ID and extract text and attachment data. Unit tests cover input selection and the response ID.
Endpoint header and documentation src/app/endpoints/responses.py, tests/unit/app/endpoints/test_responses.py, docs/devel_doc/responses.md, docs/user_doc/shields_guide.md
The endpoint uses the selected input when the header value is true. Endpoint tests cover windowed and full-history moderation. Documentation describes the header and its behavior.
The new header can reduce moderation coverage for stored or conversation-based requests, which the documentation says keep full-input moderation. Restrict the header to client-owned-context requests and fix the import ordering that fails the Ruff CI job before merging.

Docstring coverage is 100.00% which is sufficient. The required threshold is 80.00%. Docstring coverage is scoped to functions touched by this diff. Analyzed 23 functions across 7 files. (2 skipped: 2 unsupported.)
Full details: Performance And Algorithmic Complexity
Explanation
No blocking performance regression found. The new moderation window scans the input once in reverse, creates one suffix list, and extracts text and attachments with linear passes. It performs no per-item API calls, nested input-dependent loops, unbounded storage, or new list/API operation. The request body remains bounded by the existing 128 KiB limit.
Full details: Security And Secret Handling
Explanation
Passed. The PR changes the existing authenticated /responses route and adds no endpoint or authorization change. The diff contains no plaintext secrets, hardcoded tokens, SQL/command/path input sink, sensitive logging, or Kubernetes/RH Secret manifest. The new header only selects client-provided input for shield moderation, and refusal IDs are generated as modr-<uuid> and returned as response metadata.

Learning: In the lightspeed-stack repository, docstrings must use the section header name "Parameters:" (not "Args:") for function arguments, even if the project references Google Python docstring conventions. Ensure docstrings follow the project’s established "Parameters:" header format for any documented function parameters.
