docs: changelog entry for the visualizer report-quality rules and the merge-queue record

Problem

The visualizer report-quality work and the open-PR merge queue landed on dev without a changelog entry or a durable record of the merge outcomes.

Change

• CHANGELOG Unreleased: the cxc-dev-visualizer rename and REPORT-* rules, the paged template and export QA script, and the plugin-wide lessons (DEV-PRIVACY-01, FAMILY-SLOP-01 pointer, READER-DOC-05, kwrite CAT-11, uiux-design document defaults, pabcd check).
• Devlog: outcome table for feat(dev-visualizer): rename from diagram-viewer, report writing rules, A4 paged template and export QA #117, feat(subagents): add per-role first fallback dispatch #116, ci(release): guard the dispatch, fail closed on re-publish, attest the payload #112, ci: label pull requests by type and close stale needs-info issues #113, ci: delete head branches of closed-unmerged pull requests on a schedule #111, feat: add architect for domain-specific review model selection #110, fix: unify executor registration, dispatch and exit verification #91 with final heads and merge commits, plus the gh merge evidence JSON per PR.
