Removes the exact version pin for vitest, updating it to ^5.0.0 in both the root and mcp packages. Replaces the deprecated @cloudflare/vitest-pool-workers plugin with its successor, @cloudflare/vitest-plugin, which adds support for Vitest v5. All tests have been verified to pass with the new setup.
