Adds a PKGPROXY_HOST environment variable so operators can control the listen address without passing --host on the command line. Uses Cobra's Flag.Changed to distinguish an explicit --host flag from the default, ensuring --host always wins over the env var. Co-Authored-By: Claude Opus 4.7 <noreply@anthropic.com>
