docs: keep transactions and locks in actions, never in middleware - #1314
In #1304, the first fix for a race on the last admin wrapped the request in an HTTP middleware that opened a database transaction and locked the organization. That puts database rules in the HTTP layer and holds a transaction open for the whole request. The fix that shipped locks the organization inside the actions instead and checks the last admin again under the lock, like UpdateMessagePin and CreateOrganizationAdmin.
This writes that down so agents use the same pattern: transactions and row locks that guard a write live in actions, never in middleware.
Changes
app.actions: two bullets.
Transactions and row locks that guard a write go in actions, never in HTTP middleware.
When two requests can both pass the request's check before the write, the action locks the parent row, checks again and doesn't write if the check no longer holds. When the caller has to tell the user why, it throw news a dedicated exception from app/Exceptions named for the rule it protects, that the caller catches (OrganizationMustHaveAnAdmin, from feat: let organization admins manage members in the organization's settings #1304). When the request's error is enough, it skips the write silently (UpdateMessagePin). CreateOrganizationAdmin returns false instead and is named as predating the rule.
app.actions and app.http.requests: the rules that actions never check anything now name this re-check as their only exception, since it never aborts or throws a validation or HTTP exception.
Validation
Text-only change to .ai/guidelines. No code changed, so no checks were run locally. The examples were checked against production: UpdateMessagePin returns nothing, and CreateOrganizationAdmin returns false when the page was already claimed.
Independent review
A separate reviewing agent reviewed the change read-only on base 45d76afa.
First round: CODE: FAIL. The rule contradicted "actions never authorize or validate anything" and said both examples return false. It also covered read-only transactions such as the one in RelatedCommitsQuery.
SerializePushSubscriptions and SerializePushSessions already open transactions in middleware on production, and this PR leaves them unchanged. SerializePushSessions takes an advisory lock that has to cover the session save, which an action can't do. Moving them, or keeping that one as an exception, would be a follow-up.
Summary by CodeRabbit
Documentation
Clarified that concurrency-sensitive checks may be repeated in actions under a row lock and within a short transaction, with writes skipped if the condition no longer holds.
Distinguished between silently skipping a write when the request already provides an error and throwing a dedicated exception for the caller to handle.
Clarified that authorization and validation checks remain outside actions, except for rechecking a condition under a lock.

Walkthrough
The action and HTTP request guidelines describe action-level transactions and row locks for concurrency-sensitive checks. They specify under-lock rechecks and allow actions to skip writes or throw exceptions when a condition no longer holds.
The action guideline places write-guarding transactions and locks in actions. It describes rechecking conditions under a parent-row lock, then skipping the write or throwing a dedicated exception for the caller to handle. The HTTP request guideline allows an action to repeat a check under a row lock and skip the write or throw an exception for the caller to catch.
dev-null-sh/dev-null.sh#1316: Locks an organization row during an action transaction and preserves a concurrent admin check.

Review details
Before / after behavior
observed — Modified behavior in .ai/guidelines/app.http.requests.blade.php: The guideline now permits actions to recheck under a row lock to close a race; the action must skip the write or throw its own exception for the caller to catch.
observed — Modified behavior in .ai/guidelines/app.actions.blade.php: The guidelines add rules for placing write-guarding transactions and locks in actions, rechecking concurrency-sensitive conditions under a parent-row lock, and handling a failed recheck without validation or HTTP exceptions. They describe dedicated exceptions caught by callers when users must be informed, silent write skips when the request’s existing error suffices, and clarify that lock-time rechecks are the exception to the rule against action-level authorization or validation. The previous two general statements are replaced with these more specific rules.
