Summary

Fix OB Monaco completion fallback for FROM/JOIN table positions.
Support schema/table prefix completion through ODC metadata service.
Add install-time monaco-plugin-ob patch and focused regression script.

Validation

pnpm -C odc-client test:monaco-left-join
precommit type-check (tsc --noEmit)
