Fix Slack Connect image reads

Summary

• recover Slack Connect images when Slack MCP reports file_not_found
• resolve and download them through Slack Web API using the existing OAuth grant
• limit the fallback to Slack-hosted image reads with URL and size guards

Verification

• bun run format:check
• bun run lint
• bun run typecheck
• bun run test
• live read of an externally uploaded Slack Connect PNG
