[Docs] Correct recent user-facing guidance

What changed

• document the shipped Auto, Always allow, Always ask, and Disable integration-tool approval modes, including owner decisions, policy precedence, and fail-closed boundaries
• update model guidance for Claude Opus 5.5 and GPT-6 Sol/Luna, including new-connection defaults and current environment-variable examples
• explain judgment-model PR/self-review pre-screens, the Jev session-communication experiment, and judgment usage accounting
• document ordered, deduplicated task follow-ups during sandbox startup

Why this change was made

The daily public-documentation audit reviewed all 47 first-parent commits merged to develop during the exact window from 2026-09-22T03:00:45.896Z through 2026-09-23T03:00:45.896Z. These durable user-facing setup, permission, workflow, and cost-accounting behaviors were missing or stale in the shipped docs.

Impact

Public documentation now matches current shipped behavior without changing application code. Existing unmerged documentation PRs remain unshipped; in particular, this change does not copy PR #3087's superseded preview-mode approval terminology.
