[Docs] Correct model usage and Teams setup guidance

What changed

• include OpenCode Go in the Models settings guidance for providers that report current subscription usage
• point Microsoft Teams users to Settings > Personal > Linked Accounts, matching the shipped settings hierarchy

Why this change was made

A comprehensive audit of all 116 public documentation pages found these two claims had drifted from the current implementation. The corrections do not duplicate the still-open documentation audit pull requests.

Impact

Users now see the complete set of subscription providers with usage reporting and the exact path for linking a Microsoft Teams account. This is a documentation-only change; the repository-prescribed Mintlify validation and broken-link workflow passed, as did the pre-push static checks.
