- Around line 361-363: Update the affine construction to cap the spatial
dimensions at three, keeping its rank consistent with `_get_spatial_shape` and
ensuring four-dimensional ITK images produce a 4×4 affine.
- Around line 361-363: After constructing the affine from spacing, direction,
and origin, apply the LPS-to-RAS conversion when affine_lps_to_ras is true;
preserve the unconverted affine when it is false.
- Line 346: Restore the affine_lps_to_ras parameter in ITKReader._get_affine so
the existing two-argument call in ITKReader.get_data remains valid. Add get_data
coverage for both values of the flag.
- Around line 352-354: Update the ITK_SpacingBetweenSlices lookup to obtain the
metadata dictionary from sitk_img and use that dictionary for both the key check
and value retrieval, so absent keys do not call HasKey on the itk.Image object.

Read the key from the ITK metadata dictionary.
After the signature is fixed, sitk_img.HasKey(...) raises AttributeError on an itk.Image, even when the key is absent. HasKey belongs to GetMetaDataDictionary(); read the value from that dictionary too. The current ValueError handler cannot catch this failure. (docs.itk.org)

Convert two-dimensional affines when requested.
For a two-dimensional image, this condition skips conversion even when affine_lps_to_ras=True. get_data then labels the unchanged LPS affine as RAS. Apply the conversion for supported two-dimensional affines as well; keep the spatial-rank limit separate from the conversion flag. (github.com)

Call the existing affine conversion function.
For a three-dimensional image with affine_lps_to_ras=True, this branch calls self._lps_to_ras, but ITKReader defines no such method. Once the separately reported metadata lookup is corrected, get_data will raise AttributeError on this default path. Use the imported orientation_ras_lps function instead. (github.com)

Fixes #9168.
Description
This pull request fixes an issue in
ITKReaderwhere datasets with a negativeSpacingBetweenSlicesmetadata attribute resulted in an incorrect or flipped physical slice direction in the computed affine matrix.Specifically, the
_get_affinemethod now checks for"ITK_SpacingBetweenSlices"and properly accounts for negative values by ensuring the spacing magnitude is used correctly while adjusting the corresponding direction vector component, preventing slice inversion errors.Types of changes
