This removes per insert page flushes from the B tree write path.
Writes remain in the pager cache and are persisted by table_flush and table_destroy, which already own the explicit flush lifecycle. This avoids repeated disk writes and fflush calls during bulk inserts while keeping the existing persistence boundary.
The change is limited to src/btree.c.
