Add issue templates

We currently don't have issue templates in the TensorFlow Quantum repo on GitHub. After creating a lot of issues myself, I find I would actually prefer having templates to help me remember to provide information that otherwise I'll forget because I'm working too quickly.

Hey @mhucka happy to help with this if you already have a rough idea or a list of the types of issues you'd like, I have previously created several issue templates for AI/ML repos that made issuing smoother and drove deterministic outcomes and clean PRs. This would also affect some docs, probably contributing, and maybe it's worth to create an AI native workflow guide? For people using AI, instead of copy pasting issues and praying, a set of instructions could help LLMs and IDEs to easily parse and digest a repo, understand it's nuances, ripple effects, and tests before submitting prs.

Happy to start with the issue templates alone, and implement the rest in a follow up, or in the same PR. Let me know what you think.
